New Patient Application Requirements:

• Valid Drivers License / ID Card 

• Email Address (This is how OMMA will always contact you)

• Doctor Recommendation.  If your address on your doctor recommendation does not match your current ID you will need to bring a utility bill in your name to prove residency.

• Sooner Care, Medicaid or Medicare Card

• Debit Card / Credit Card for state fee.

• Patients w/o Sooner Care, Medicaid or Medicare $104.50

• Patients with $22.50

Patient Renewal Application Requirements:

• All ID / Documents Required for a New Patient Application

• New Doctor Recommendation with valid date (within 30 days).

• Original Email Address that was used when you applied for your card. If you do not remember the email address, please contact OMMA at (405)522-6662.
